Essentials of diagnosis in TCM:
1. Medical history or primary lesion with pulmonary tuberculosis, cancer, etc. More is in the unilateral side. Some are in the bilateral sides.
2. Distension and fullness at the side of chest and hypochondrium, hypochondriac pain when coughing, accompanied by fever, short breath, prefer to lie on the side.
3. Satiation at one side of the thorax, weakness of breathing activity, vocal fremitus, and breath sounds. Percussion shows voiced sound or flatness.
Differentiation and analyse
Hypochondriac pain: dry cough, obvious hypochondriac pain, pleural friction sound heard clearly, not obvious hydrops in the chest. These are the manifestations at the former and later stage.
Lung fever: cough, expectoration, shortness of breath, rale in the lung, no pleural effusion.
In TCM, a disease or a symptom might be caused by one pathogenic factor, even two or three pathogenic factors. When diagnosing a disease or a symptom, TCM doctors must follow the principle of "Syndrome Dfferentiation", and then "Suit the Remedy to the Case".
