Outline:
Intertrochanteric fracture is the fracture occurring between the greater and lesser trochanters, also called intertrochanter fracture of femur. It is often seen in old people, usually caused by indirect violent force. The typical gesture of injury is just the same as that in the fracture of neck of femur. According to the direction and location of fracture line, it may be classified into three types of anterograde intertrochanteric, retrograde intertrochanteric and subtrochanteric fractures. Fracture of trochanter is commonly comminuted because of its loose and fragile substances. The fracture can be healed generally due to rich supply of blood for trochanter, with a good prognosis.
Reduction
It is just as the same as that for fracture of neck of femur.
Fixation The patients with no displacement may wear T-shaped shoes, or receive skin traction with weight of 3 to 5 kg. The duration of fixation is 6 to 7 weeks. For those with displacement, the skeletal traction through tubercle of tibia or supracondyle of femur should be applied, and the weight for traction is 6 to8 kg. The affected limb should be fixed in extortion and neutral position for 8 weeks (for stable fracture) to 10 weeks (for unstable fracture).
Functional exercise:
During the period of fixation, the patient is asked to avoid crossing his legs, taking lateral recumbent position, or cross-legged sitting, and to often practice muscular activities of the affected limb and general activities. After removing the traction, the patient may do walking exercise supported by a pair of crutches with no weight bearing. The patient should not bear weight until the fracture is healed through confirmation by roentgenogram.
Herbal therapy:
It is just as the same as that for fracture of neck of femur.
