Syndrome differentiation and treatment
The main therapeutic principle of ascariasis lies in expelling and destroying ascarides. According to the clinical manifestations, different methods may be adopted of calming ascarides, dispersing ascarides and expelling ascarides. After the anthelmintic treatment, regulating the spleen and stomach is accordingly cooperated.
1. Ascarides lodging in the intestine
Main symptoms and signs: Intermittent pain around the navel without obvious tenderness but with cord-shaped sensation when pressed, gastric discomfort with acid regurgitation, abnormal appetite, restless sleep, teeth grinding during sleeping, frequent salivation of lucid saliva, or nausea and vomiting, or vomiting of ascarides.
Therapeutic methods: The main methods are expelling and destroying ascarides while regulating the spleen and stomach is accordingly cooperated.
2. Ascarides reaching the gallbladder
Main symptoms and signs: Symptoms of common intestinal roundworm disease exist. The patient has sudden severe paroxysmal colic of the region below xiphoid process and the upper right abdomen which is so severe that the patient cries, rolls about, bends over and supports the painful site with the fist as well as restlessness, cold limbs and sweating. The intermittent pain is severe and mild alternatively. Abdominal pain is not obvious in infantile patients during the remission time. Or concurrent nausea and vomiting and occasional vomiting of ascarides may occur. Repeated or persistent attack often exist; sometimes, chills, fever, even jaundice in severe cases are accompanied by yellowish and greasy fur, slippery and rapid or taut and rapid pulse.
Therapeutic methods: Calming ascarides to check pain, followed by expelling ascarides.
3. Ascarides caking in the intestine
Main symptoms and signs: General symptoms of intestinal roundworm disease exist. The patient has sudden severe paroxysmal abdominal pain mainly around the navel, accompanied by frequent vomiting, possible vomiting of roundworms and no defecation in general. However, defecation is still possible in the case of high intestinal obstruction. Indefinite cord-shaped mass may be felt. The mass has activity when pressed. The abdomen is mostly soft and free from marked tenderness.
Therapeutic methods: Promoting qi flow, dredging the fu organ to disperse ascarides, followed by expelling ascarides.
