Chinese medicine Treatment for Allergic Purpura

The therapeutic principles of purpura are removing toxin to eliminate ecchymoses in the case of sthenia; tonifying qi to control blood and nourishing yin to remove fire in the case of asthenia.
Syndrome differentiation and treatment:
1. Wind and heat impairing collaterals     
Main symptoms and signs: Abrupt onset, fever, aversion to wind, cough, sore throat, etc. at the beginning, followed by skin purpura mainly appearing on lower limbs and buttocks with bright red colour, concurrent itching, abdominal pain, swelling and pain of joints, hematuria, etc., red tongue with yellowish and thin fur, floating and rapid pulse.
Therapeutic methods: Expelling wind and clearing away heat, cooling blood to calm collaterals.

2. Bleeding due to blood heat 
Main symptoms and signs: Abrupt attack, skin petechiae and ecchymoses, even patches of petechiae and ecchymoses with purplish red colour, concurrent dry mouth with preference to cold drink, fever, red
Therapeutic methods: Clearing away heat and removing toxin, cooling blood and resolving ecchymoses.

3. Failure of qi to keep blood circulating intravascularly     
Main symptoms and signs: Recurrent purpura, purplish ecchymoses and petechiae, occasional nasal bleeding, gingival bleeding, lustreless complexion, dizziness, palpitation, listlessness, short breath, poor appetite, pale tongue with thin fur, thready and forceless pulse.
Therapeutic methods: Strengthening the spleen and nourishing the heart, supplementing qi to control blood.

4. Hyperactive fire due to yin deficiency    
Main symptoms and signs: Intermittent onset of purpura, concurrent lingering nasal and gingival bleeding, low fever, night sweat, vexation, poor sleep, dry mouth and throat, constipation, dark urine, red tongue with scanty fluid and a little fur, thready and rapid pulse.
Therapeutic methods: Nourishing yin to remove fire, cooling blood to check bleeding.
