To make the diagnosis of edema, it is essential to clarify whether the complaint is edema of the yang type or edema of the yin type. Usually the yang-type edema is characterized by a short duration, a rapid development, more pronounced edema on the head and face with the skin shiny and thin. However, the yin-type edema is marked by a longer duration, a gradual development, more pronounced pitted edema of lower limbs, sallow and grayish skin.
The treatment for the yang-type should aim at eliminating pathogenic factors, with such methods as diaphoresis and diuresis. When necessary, purgation should be used. For the yin-type, it is advisable to support healthy qi to eliminate pathogenic factors, for instance, to promote diuresis by invigorating the spleen and warming the kidneys. If edema is persistent, promoting diuresis by activating blood circulation and dissipating blood stasis is recommended as the accessory treatment.
