Diagnostic Essentials for Gonorrhea in Chinese medicine:
1.Simple Gonorrhea
(1) Acute Gonorrheal Urethritis
The incubation period ranges from 2 - 10 days, 3 - 5 days on average. The typical symptoms are red tumefaction and pruritus in the meatus urinarius, with thin mucus or mucous purulent secretion, aggravated after 24 hours, followed by painful urination (burning sensation), discharge of tenacious and dark yellowish pus, also urgent and frequent urination and accompanied by general discomfort, red edema in the meatus urinarius. In severe cases, the mucous membrane of the urethra rolls outwards and there is often painful erection of the penis at night. About 25 % patients only discharge thin and slightly purulent secretion, which is difficult to be identified from non-gonorrheal urethritis (NGU) by the naked eyes.
(2) Female Urinosexual Gonorrhea
1) Gonorrheal Cervicitis: After infection in the females, the incubation period is difficult to be defined.There has already been 10 days in infection, when the local symptoms are found in most cases. The uterine neck is the most common region of the primary infection, manifested by red tumefaction in different degrees, tenderness and purulent secretion.
2) Gonorrheal Urethritis: It often occurs 2 - 5 days after sexual intercourse, manifested by frequent urination, painful urination, red tumefaction in the meatus urinarius, discharge of purulent secretion. The symptoms are often mild in males and there can also be no symptom.
3) Gonorrheal Anorecfitis, Gonorrheal Pharyngitis and Gonorrheal Conjunctivitis.
2. Combined Gonorrhea
(1) Male Gonorrheal Complication If acute anterior urethritis in males is not treated completely, inflammation can spread to the posterior urethra to cause posterior urethritis, acute or chronic prostatifis, etc., often complicated withseminal vesculitis, epididymitis, COwperitis and tysonitis, etc. The symptoms attack repeatedly to cause urethrostenosis after scars are formed. The stricture or obstructionof the spermatic duct can happen in some cases, and secondary infertility can ensue.
(2) Female Gonorrheal Complication Mostly, if gonorrheal cervicitis is not treated promptly, inflammation can go up to cause infection, leading to complications of endometritis, salpingitis, or tubal abscess, or ovary abscess, pelvic inflammation, perianal inflammation, and also bartholinifis, skenitis or abscess of the paraurethral gland. Due to repeated attack of inflammation, contraction of scars can lead to stenosis or obstruction of the fallopian tube, and exfetation or sterility.
3. Disseminated Gonorrheal Infection
It occurs mostly frequently in the women during the periods. Gonococcus spreads to the whole body via blood circulation to cause bacteremia, manifested by high fever, chills and skin rashes. The skin rashes, which are pustules, happen often near the joints of the fingers and ankle, in scattered distribution. Arthritis, tenosynovitis, meningitis, endocarditis , pericarditis, pleurisy and pneumonia can also occur.
