Treatment Based on Syndrome Differentiation
1. Syndrome of Qi Stagnation in Intestines
Main Symptoms; It is similar to early intestinal obstruction and adhesive intestinal obstruction in Western medicine, manifested by paroxysmal distending pain in the abdomen, with distending sensation worse than pain, intermittent appearance of mass in the abdomen, drum-like abdomen in percussion, slight tenderness, accompanied by nausea, vomiting, no flatus and defecation, slightly red tongue, thin and whitish coating, and wiry pulse.
Therapeutic Methods: Promote flow of qi, disperse accumulation, dredge the interior for purgation.
2. Intestinal Stagnation and Accumulation Syndrome
Main Symptoms: It is similar to various types of intestinal obstruction with disturbance of blood circulation of different degrees or tumorous intestinal obstruction,manifested by serious abdominal pain, with pain worse than distending sensation, fixed location of pain, obvious palpable mass in the abdomen, aggravated by pressure, coffee-like vomiting substance sometimes, accompanied by dark red tongue, or with purple spots, yellowish and greasy coating, and hesitant pulse.
Therapeutic Methods: Disperse stagnation, promote flow of qi and dredge the interior for purgation.
3. Syndrome of Obstruction due to accumulation of Worms
Main Symptoms: This syndrome is often seen in children with a history of roundworm, manifested by paroxysmal abdominal pain, with constant attacks, palpable roundworm mass in the abdomen, intermittent flatus, pale tongue, thin and whitish coating, and wiry pulse.
Therapeutic Methods: Expel worm, circulate qi and dredge Fu organs for purgation.
4. Syndrome of Accumulation of Dryreesin IntesUnss
Main Symptoms: This syndrome is often seen in the old and feeble patients with a history of habitual constipation, manifested by difficult defecation and constipated stool, progressive abdominal distension, abdominal pain, palpable feces mass in the left lower abdomen, accompanied by poor appetite, dry tongue with scanty fluid, thick and greasy coating, thready and rapid pulse.
Therapeutic Methods: Nourish yin, moisten the intestine and dredge the interior for purgation.
5. Syndrome of Retention of Water and Obstruction of Dampness
Main Symptoms: This syndrome is similar to various types of intestinal obstruction complicated with accumulation of fluid in the intestines, manifested by progressively aggravated abdominal pain, constant borborygmus, abdominal distension aggravated by pressure, accompanied by nausea, vomiting, thirst without preference for drinks, no flatus and no defecation, scanty urine, reddened tongue, whitish and greasy coating, slippery and rapid pulse.
Therapeutic Methods: Circulate qi, expel water and dredge Fu organs for purgation.
6. Syndrome of Obstruction due to Retention of Food
Main Symptoms: Sudden seizure of serious abdominal pain with restlessness after a hearty meal, over exertion or fierce sports, manifested by constant pain, paroxysmal aggravation, frequent vomiting, abdominal distension aggravated by pressure, no flatus and no defecation, pale tongue, yellowish, thick and greasy coating, slippery and rapid pulse.
7. Syndrome of Phlegmon Accumulation in Intestines
Main Symptoms: This syndrome is similar to strangulafied intestinal obstruction of the later stage and toxic intestinal paralysis, manifested by abdominal pain, abdominal distension, and abdominal fullness aggravated by pressure, nausea, vomiting, no flatus, no defecation, accompanied by high fever, restlessness, spontaneous perspiration, dry mouth and tongue, even cold limbs, reddened tongue, yellowish, thick and greasy coating, wiry and rapid pulse or deep, thready and rapid pulse.
Therapeutic Methods In accordance with the principle to treat the symptoms at the acute stage, if no contraindication for surgery, emergent surgery should be given immediately.
