Syndrome differentiation and treatmentThe main symptom in pelvic inflammation is lower abdominal pain. So syndrome differentiation should be done in light of the nature and degree of pain as well as the accompanied symptoms. Usually the syndrome with sudden onset and severe pain accompanied by chills and high fever pertains to heat and sthenia: while the syndrome with incomplete treatment, lingering duration or slow onset is marked by a mixture of cold and heat as well as asthenia and sthenia.
The frequent manifestations at the acute stage are accumulation and exuberance of damp-heat in the lower energizer which should be treated mainly by clearing away damp-heat and removing toxin to resolving stagnation; the usual manifestations at the chronic stage are qi stagnation and blood stasis or asthenia of both the spleen and kidney, the former should be treated by promoting qi flow and activating blood, while the latter by dealing with both the asthenia and sthenia. For the treatment of frequent relapse and abscess or formation of cyst that cannot be effectively with drugs, operation has to be resorted to.
(1) Syndrome of accumulation and exuberance of damp-heat
Main symptoms: Fever, aversion to cold, swearing, unpressable pain in the lower abdomen and sides of the lower abdomen, yellowish and pus-like leukorrhea, red tongue with yellow and greasy fur as well as slippery and rapid pulse.
Therapeutic methods: Clearing away heat and draining dampness, resolving toxin and stasis.
(2) Syndrome of qi stagnation and blood stasis
Main symptoms: Stabbing pain in the lower abdomen and sides of lower abdomen, even formation of mass,aching pain in the loins, irregular menstruation, profuse leukorrhea, lassitude, purplish tongue or with ecchymoses, whitish thin tongue fur and taut and unsmooth pulse.
Therapeutic methods: Regulating qi, activating blood and stopping pain.
(3) Syndrome of asthenia of both spleen and kidney
Main symptoms: Dull pain in the lower abdomen and both sides of lower abdomen, even aching pain in the lumbosacral region, profuse and thin leukorrhea, spiritual lassitude, cold limbs, loose stool, thin and greasy tongue fur as well as thin and deep pulse.
Therapeutic methods: Nourishing the kidney and strengthening the spleen, harmonizing the collaterals and stopping pain.
