Ingredients:
No. 1 Shuiniujiao (Cornu Bubali) (concentrated powder) 30 g (originally Xijiao (Cornu Rhinocerotis)] ;
No. 2 Zhusha (Cinnabaris) 30 g;
No. 3 Xionghuang (Realgar) 30 g;
No. 4 Daimao (Carapax Eretmochelydis) 30 g;
No. 5 Hupo (Succinum) 30 g;
No. 6 Shexiang (Moschus) 7.5 g; (Generally use artificial one)
No. 7 Bingpian (Borneolum) 7.5 g;
No. 8 Niuhuang (Calculus Bovis) 15 g;
No. 9 Anxixiang (Benzoinum) 45 g.
Administration:
Grind the above into fine powder and mix the powder with honey to make pills weiging 3 g each; take one pill each time and once daily with warm boiled water.
Actions:
Clearing away heat and dissolving turbid and toxic substances to induce resuscitation.
Clinical Application:
This recipe is indicated for syndrome of phlegm-heat retaining in the pericardium, marked by coma, delirium, dysphoria, abundant expectoration, harsh respiration, reddened tongue with thick greasy yellowish fur, smooth and rapid pulse. It is applicable to epidemic cerebrospinal meningitis, epidemic encephalitis B, toxic dysentery, uremia, cerebrovascular accident, and hepatic coma and others which belong to phlegm-heat retaining in the pericardium. In case of deficiency of vital-qi with feeble pulse, take the pills with the decoction of Renshen (Radix Ginseng).
Elucidation:
The syndrome is caused by retention of phlegm in the pericardium due to excessive heat. It should be treated by clearing away heat and removing phlegm. Ingredients No. 1 and No. 6 act as monarch drugs, and the combined application of both constitutes the result of clearing away heat and inducing resuscitation. Ingredients No. 7 and No. 9 are aromatic in flavor and capable of restoring consciousness, eliminating turbid substances and promoting the action of No. 6.
Ingredients No. 8 and No. 4 can clear away heat and toxins, besides No. 8 can also eliminate phlegm to induce resuscitation and relive convulsion or spasm, and promote the effect of No. 1 in clearing away heat from blood. These four ingredients play the role of minister drug. Serving as adjuvant drug, No. 2 and No. 5 can relieving palpitation and tranquilize mind, and No. 3 eliminate phlegm and toxins. Gold and silver foils were used in the original recipe, which were meant to strengthen the effect of tranquilizing mind. But they have been deleted presently from Chinese Pharmacopoeia.
This recipe and Angong Niuhuang Wan, Zixue (Powder) are collectively named "Three Treasures", which share similar functions and chief indications and possess their respective advantages when their therapeutic effects are analyzed in detail. Angong Niuhuang Wan is good at clearing away heat and toxins and fit for cases with marked high fever and delirium as seen in unconsciousness due to heat. Zixue (Powder) excels in relieving convulsion and spasm and is fit for cases with convulsion and clonus. Zhibao Dan is effective in inducing resuscitation and eliminating turbid substances, thus suitable for cases with severe coma and thick greasy fur. They should be applied according to the condition of illness.
Cautions:
This recipe comprises many drugs of aromatic, pungent and dry nature, which are likely to consume and impair yin and body fluid. Therefore, it should be used with great caution in patients with excessive deficiency of yin.
