Herbal Therapy
1) Internal Medicine
(1) Syndrome of qi and blood stagnation:
Therapeutic methods: Promoting blood flow to remove the stasis, activating qi flow to stop pain.
Recipe and herbs: Modified Taohong Siwu Decoction.
(2) Syndrome of collateral disharmony:
Therapeutic methods: Relaxing tendons and activating collaterals.
Recipe and herbs: Modified Huoxue Shujin Decoction.
External Medicine
1) In the early stage, Xiaoyu Zhitong Paste or Sanse Paste is externally applied. In the late stage, fumigation and bathing of the diseased part with Haitongpi Decoction is taken.
Fixation
For the acute cases, splints or a plaster support is used to fix the sick limb in a position of the knee bent at the angle of 100. The patient should not be allowed to walk and carry weight. The course is 3 or 4 weeks.
3) Get 500g of salt and stir-fry it hot; add 50g of sliced ginger; wrap them with a piece of cotton cloth and apply it on the effected area; a good effect will be gained.
4) In the early stage, the injured limbs should be avoided moving of and raised up; apply the Chinese herbal medicine of Jiegu San on the effected area to eliminate the swelling and pain.
(2) Acupoints Massage
(1) Tendon-smoothing manipulation
For the case of acute injury, one time of passive extension and flexion of the knee joint should be done. The patient takes a supine position with his sick limb relaxed. The operator massages the tender point with his left thumb, holds the ankle with his right hand, slowly flexes the knee joint while making intorsion and extorsion of the leg and then straightens it out. This can relieve local pain.
For the chronic cases, local massage is done every day or every other day. First the operator presses the tender point on the margin of the joint with his thumb, then massages and kneads on around the tender point. (2) Massage: To activate blood circulation to remove stasis, eliminate swelling to stop pain. Common manipulation: Supporting by the arm, kneading, touch lightly as with finger, etc.
Manipulation:
1) Early stage: The patient lies in a supine position. The operator takes manipulation of supporting by the arm, kneading, etc. on around the knee and the forepart of thigh.
2) The Bending and Extending method can be taken to free from the interlock condition for the patients suffering from the "interlock sign": The patient lies in a supine position and bent knee and hip of 90 degree. An assistant holds the lower of thighbone and the operator holds the ankle; the both men take traction face to face; the operator can make several times of extorsion of the leg to bend it to the maximum limit; and then straighten it out. This can free from the interlock condition.
3) Press the points of Yanglingquan (GB 34), Yinlingquan (SP 9), Xiyan (EX- LE 4, 5), Zusanli (ST 36), Weizhong (BL 40), Xuehai (SP 10), etc.
4) Local massage: The patient lies in a supine position. The operator presses the tender point on the margin of the joint with his thumb, then massages and kneads on around the tender point.
(3) Cupping Therapy
Acupoints selected: Liangqiu (ST 34), Xiyan (EX- LE 4, 5), Xiyangguan (GB 3), Yanglingquan (GB 34), Weizhong (BL 40), Ququan (LR8) and Ashi Points.
Manipulation:
1) Fire Cupping Method: The cup is sucked in place for 15 minutes each time for each point; once a day and 10 days is a course of treatment. The second course should be taken after a rest of one week.
2) Blood-letting Puncture and Cupping Method: Take cupping after pricking for blood-letting with the triangle-edged needle.
