Acupuncture & moxibustion for prostatitis

(1) Body Acupuncture
Acupoints Selected: Zhongji (CV 3), Ciliao (BL32), Sanyinjiao (SP6) and Yinlingquan (SP9).   
 
Modification: For downward migration of damp-heat, Qugu (CV2), Zhibian (BL 54), Pangguangshu (BL 28) and Sanjiaoshu (BL22) are added; for accumulation of heat-toxin, Quchi (LI 11) and Hegu (LI 4) are added; for stagnation of cold in liver vessels, Dadun (LR 1 ) and Taichong (LR 3) are added; for asthenia of liver and kidney yin, Guanyuan (CV 4), Fuliu (KI7), Taixi (KI3), Shenshu (BL23) and Ganshu (BL 18) are added; for asthenia of spleen and kidney yang, Mingmen (GV 4 ), Pishu (BL20), Shenshu (BL23) and Zusanli (ST 36) are added; for qi stagnation and blood stasis, Taichong (LR 3) and Geshu (BL17) are added; for hematuria, Xuehai (SP 10), Geshu (BL17) and Zhibian (BL 54) are added; for seminal emission and spermatorrhea, Jinggong (Ext.), Guanyuan (CV 4) and Dahe (KI 12) are added.    
   
Performance: Urine is discharged completely, then Zhongji (CV 3), Guanyuan (CV 4) and Qugu (CV2) are needled vertically for 1.5 - 2 cun to direct the needling sensation to the perineum; Ciliao (BL32) and Zhibian (BL 54) are needled 1.5 - 3 cun toward the bladder to direct the needling sensation to the lower abdomen and perineum; the other acupoints are needled with routine procedure. The needling technique used is mainly for reducing purpose. For the treatment of asthenia of the spleen and kidney yang, reinforcing needling technique is used together with moxibustion. For the treatment of asthenia of liver and kidney yin, mild reinforcing and reducing needling technique is used.    

(2) Ear Acupuncture
Acupoints Selected:
Angle of Cymba Concha (CO8), Kidney (CO10), Bladder (CO9), Liver (CO12), Triple Energizer (CO17), Ear Shenmen ( TF4 ), Pelvic Cavity ( TF5 ), Sympathetic Nerve (AH6a), Endocrine (CO18) and Adrenal Gland (TG2p).   
Performance: Each time 3 - 5 acupoints are selected with normal procedure. For the treatment of acute prostatitis, strong stimulation (with bloodletting from the tip of ear) is used. The needles are retained for a longer (45 - 60 minutes) time and manipulated at intervals. For the treatment of chronic prostatitis, moderate stimulation is used. The needles are retained for 20 minutes and the needling is done once a day.

(3) Acupoints Selected:
Zhongji (CV 3), Yinlingquan (SP9), Sanyinjiao (SP6), Huiyin (CV 1). Jinggong (Extra.) is added if there is seminal emission.
Action: Clearing away damp-heat by promoting diuresis.

Elucidation:
The disease fails into the category of "stranguria syndrome", "whitish sperm" or "turbid sperm" etc. in TCM. Chronic prostitis can be evolved from acute prostitis which is not be thoroughly cured: the left damp-heat obstructs lower-jiao, which causes prostitis to recur. Or the occurrence of it can be due to asthenia of kidney-qi caused by usual excessive sexual intercourse, and invasion as well as accumulation of pathogenic damp-heat at the perineum. Or it can result from usual drinking too much alcohol which impairs the function of the spleen, interior generation and stagnation of damp-turbid at lower-jiao, and stagnation of qi as well as blood. The disease is deficiency in origin and excess in superficiality.

Hence, Zhongji (CV 3), the convergent acupoint of the conception vessel and the foot triple-yin meridians, and Yanglingquan (GB 34), the He-Sea point of the spleen meridian, are distally-proximally associated to clear away damp-heat, to regulate lower-jiao. Sanyinjiao ( SP6 ), the convergent acupoint of the foot triple-yin meridians, is selected to remove dampness by diuresis, to nourish yin. Huiyin (CV 1 ), the convergent acupoint of the conception, governor and thoroughfare vessel, is proximally selected to clear away damp-heat, to relieve inflammation to arrest pain. Jinggong (Extra.) can be needled to tonify kidney, to control nocturnal emission, and to arrest spontaneous emission.

Indication:
Chronic prostitis (Type of down-ward flow of damp-heat). The main manifestations are oliguria with urgent and painful urination, dark yellowish urine, or hematuria if severe, dry and bitter mouth, damp-heat vulva, pain of perineum, sometimes seminal emission, reddish tongue with yellowish and greasy fur, slippery and rapid pulse.
Performance: Zhongji (CV 3) is needled vertically 1 - 1.2 cun with the reducing method achieved by lifting-inserting manipulated on the needle.

Yinlingquan (SP 9) is needled vertically 1 - 1.5 cun with the reducing method achieved by lifting-inserting manipulated on the needle.
Sanyinjiao (SP6) is needled vertically O. 8 - 1.2 cun with the reducing technique manipulated first and the reinforcing technique next on the needle.
Huiyin (CV 1 ) is needled obliquely bilaterally to make the acuesthesia transmit to the vulva.
Jinggong (Extra.) is needled obliquely 0.5 - 1 cun toward the spinal column with the reinforcing method achieved by lifting-inserting manipulated on the needle.
The above needles are all retained for 20 - 30 minutes. The treatment is given every other day.
