1. The commonly-seen syndromes of acute nasosinusitis include three types, wind and heat of lung meridian, stagnated heat of the gallbladder as well as dampness and heat of the spleen and stomach.
(1) Syndrome of wind and heat of lung meridian:
Main symptoms and signs: Nasal discharge is yellowish or whitish and sticky, with profuse quantity. Nasal obstruction is intermittent or persistent, with hyposmia. Examination shows congestion of the nasal mucosa, enlargement of two inferior nasal conchae, pus in the nasal meatus, tenderness of the glabellum or zygomatic parts. The general symptoms include fever, aversion to cold, headache, thoracic tightness, cough and profuse sputum.The tongue is red with yellowish coating and the pulse is floating and rapid.
(2) Syndrome of stagnated heat of the gallbladder:
Main symptoms and signs: Nasal discharge is turbid, sticky and thick like pus with profuse quantity and yellowish colour. It flows downwards from the upper part of the nasal cavity, with foul odour. The patient has hyposmia, swelling of the nasal mucosa with reddish colour. Headache is severe with marked tenderness on the glabellurn and zygomatic parts. The patient may have the accompanying symptoms of fever, bitter taste in the mouth, dizziness, tinnitus, less sleep, dream-disturbed sleep,restlessness of mind and irascibility. (3) Syndrome of dampness and heat of the spleen and stomach:
Main symptoms and signs: Nasal discharge is yellow, turbid and profuse, flowing out slowly from the upper part of the nasal cavity. Nasal obstruction is severe and persistent with anosmia. The patient has red swelling of the nasal mucosa, with distending pain, and the swelling is especially severe. The general symptoms include dizziness, heavy sensation of the head, severer headache, lassitude, epigastric and hypochondriac distension and tightness as well as dark urine. The tongue is red with yellowish and greasy coating The pulse is soft or slippery and rapid.
2. The commonly-seen syndromes of chronic nasal sinusitis include two types: asthenic cold of the lung qi and qi deficiency of the spleen. The syndrome of asthenic cold of the lung qi is characterized by pale Main symptoms and signs: complexion and aversion to wind. Treatment should be warming and invigorating the lung qi. The syndrome of qi deficiency of the spleen is marked by anorexia, abdominal distension as well as clear and thin nasal discharge with profuse quantity. Treatment should lie in strengthening the spleen and invigorating qi.
(1) Syndrome of asthenic cold of the lung qi:Main symptoms and signs:
Main symptoms and signs: Nasal discharge is whitish and sticky. Nasal obstruction is severe or mild, with hyposmia. Internal nasal sarolemma is reddish and swollen,with hypertrophy of conchae. Nasal obstruction and discharge worsen when the nose is stimulated by cold, etc.The general symptoms include dizziness, cerebral distension, cold body and limbs, short breath, fatigue and cough with sputum. (2) Syndrome of qi deficiency of the spleen:
Main symptoms and signs: Nasal discharge is whitish, sticky and profuse, but free from foul odour. Nasal obstruction is severer with hyposmia. Internal nasal sarolemma is reddish or deep red with severer swelling. The general symptoms include fatigue of limbs, anorexia, abdominal distension, loose stool and sallow complexion. The tongue is pale with whitish thin coating and the pulse is slow and weak.
