Although the diseased part involved in epigastric pain is the stomach, it is closely related with the liver and the spleen. This problem may be caused by invasion of exogenous pathogenic cold, overeating of raw and cold food, an attack on the stomach by liver qi or weakness of the spleen and stomach. Clinically, it is divided into excess type and deficiency type. In excess condition, the pain is very severe and aggravated by pressure, especially after a meal; in deficiency condition, the pain is dull, alleviated by warmth and pressure, and is aggravated by an empty stomach. The principle of treatment is to regulate the spleen and stomach, and promote the flow of qi to stop pain with Zhongwan (Ren 12), Pishu (BL20), Neiguan (PC6) and Zusanli (ST 36) as the main points. For pathogenic cold, add moxibustion; for attack on the stomach by liver qi, add Ganshu (BL18), Taichong (LR 3); and for weakness of the spleen and stomach, add Weishu (BL21), Zhangmen(LR13) and Sanyinjiao (SP6). Stomachache is more apparent in acute and chronic gastritis, gastric and duodenal ulcer, and stomach neurasthenia.
