TCM Diagnosis for Vertigo

It is essential to differentiate vertigo of excess type from that of deficiency type. The excess type is usually caused by both hyperactivity of liver yang and retention of turbid phlegm and induced by emotional depression and anger; it is characterized by a short course or intermittent episodes, severe dizziness with a rotatory feeling accompanied by nausea, vomiting and salivation and often found in those with strong physique. Deficiency of blood or insufficiency of kidney essence often contributes to deficiency type which is characterized by a long course, intermittent or persistent occurrence and is induced or aggravated by overstrain; it is also manifested as dizziness and heavy sensation in the head with no rotatory feeling, and general debility as well.

Essentials for Diagnosis 
(1) The chief manifestations are dizziness and blurred vision. The mild case can be relieved by closing one's eyes; the severe case is characterized by a rotatary feeling like sitting in a sailing boat or moving car, even fainting.

(2) There may be accompanying symptoms such as nausea, vomiting, nystagmus, tinnitus, deafness, sweating and pale complexion.

(3) Slow onset, gradual aggravation, or repeated attacks.

(4) To make a clear diagnosis, hemoglobin, red cell count, blood pressure, electrocardiogram, cervical vertabrae X-ray radiography, Doppler transcranial ultrasonic examination, CT and MRI examinations may be done.
