Decoction of Trichosanthes and Macrostem with Liquor

Source: Jingui Yaolue (Synopsis of Prescriptions of the Golden Cabinet).

Ingredients:
No. 1 Quangualou (Fructus Trichosanthis) 12 g;
No. 2 Xiebai (Bulbus Allii Macrostemi) 9 g;
No. 3 Liquor 30 - 60 ml.

Administration:
Put in proper amount of water to decoct the above drugs for oral use.
 
Actions:
Activating yang to remove stagnation, promoting the circulation of qi to eliminate phlegm.

Clinical Application:
This recipe is indicated for chest bi-syndrome (obstruction of qi in the chest),marked by vague pain over the chest, chest pain radiating to the back if severe, dyspnea, cough with sputum, shortness of breath, greasy whitish fur, deep and taut or tense pulse. It is applicable to angina pectoris of coronary heart disease, non-suppurative costochondritis, intercostal neuralgia, and other diseases, which pertain to obstruction of the chest-yang by phlegm and are chiefly manifested as chest pain, shortness of breath, whitish fur and taut pulse.

In case of excessive phlegm with chest fullness and thick greasy fur, add Banxia (Rhizoma Pinelliae) to dispel phlegm, forming another recipe entitled Gualou Xiebai Banxia Tang. In case of excessive stagnation of qi with subjective sensation of qi rising upward, add Houpo (Cortex Magnoliae Officinalis), Zhishi (Fructus Aurantii Immaturus) and Guizhi (Ramulus Cinnamomi) to regulate the flow of qi and check the adversely rising qi, which is known as Zhishi Xiebai Guizhi Tang. In case of dominant cold with manifestations as chest pain radiating to the back and deep slow pulse, add Ganjiang (Rhizoma Zingiberis), Guizhi (Ramulus Cinnnamomi) and Fuzi (Radix Aconiti Praeparatae) to activate yang and dispel cold.
 
Elucidation:
The syndrome is caused by deficiency of chest-yang, leading to accumulation of phlegm in the interior and disorder of qi activity. It should be treated by activating yang, removing obstruction, promoting flow of qi and eliminating phlegm. The first ingredient is the monarch drug, which resolves phlegm, promotes the circulation of qi, and relieves stuffiness of the chest. No. 2 acts as minister drug, which activates yang and relieves stagnation, promotes the flow of qi and relieves pain.

These two ingredients supplement each other and are used as primary drugs for treating obstruction of qi in the chest, with the one removing stagnation of phlegm, and the other activating yang. Pungent and warm in nature, the last ingredient is capable of regulating qi and blood, performing its efficacy to strengthen the effect of activating the chest yang, and serves as adjuvant drug.

Cautions: If liquor is intolerable for the patient, it may either be applied with reduced amount or disused.
