Herbal Therapy
1) Internal Medicine
(1) Syndrome of Wind and Heat Invasion of Eyelid
Therapeutic Method: Removing pathogenic wind and heat.
Prescription: The modified Yinqiaosan Decoction
Syndrome of Excessive Heat in Spleen and Stomach
Therapeutic Method: Clearing the spleen to disperse pathogenic wind and heat.
Prescription: The modified Chufeng Qingpi Decoction.
Syndrome of Stagnation of Blood Heat
Therapeutic Method: Dissipating stasis by cooling blood.
Prescription: The modified Guishao Honghuasan Decoction.
2) External Medicine
(1) Eye Drops
a. Coptis and Watermelon-Frost Eye Drops. The eye is dropped three or four times every day for trachoma in mild cases.
b. Huatiedan Eye Drops. The eye is dropped three times every day for trachoma in mild and severe cases.
c. Xihuangsan Eye Drops. The eye is dropped once or twice every day for trachoma of various syndromes.
(2) Rubbing Therapy
The cuttle-bone-rod-rubbing therapy can be used to treat trachoma in severe cases with papillae, follicles and granules of the conjunctiva. Procedure: cut the cuttle-bone with a knife into the duck-billed rod. After sterilizing it in the boiling decoction of coptis root, take it out and dry up for later use. If Dengxincao (Medulla Junci) is used, first cut it to small pieces and sterilize with the above method. In the operation, grip the piece with a pair of tweezers. After the surface anesthesia of the eye to be operated on with 0. 5% to l% of dicaine and the cleaning of the conjunctival sac, turn the upper and lower eyelids from inside out with the operator's left hand to let the fornix be fully exposed, and then, with the operator's right hand holding the cuttle-bone-rod, gently and swiftly rub the part of palpebral conjunctiva covered with granules back and forth for many times till punctate hemorrhage appears. When the rubbing is finished, wash the conjunctival sac with normal saline or antibiotic. Finally smear the eye with some eye ointment. The operation should be performed once every five days, or more times depending on the conditions of the disease.
During the operation, special attention must be paid to the sterilization and the gentle maneuver in order that the cornea might not be injured. (3) Sickle-washing Therapy
The sterilized filiform needle is used. Or dried clean Longxucao (Culmus1et Folium Poae Sphondylodis), soaked for 24 hours in 10% of boiled decoction of coptis root, is prepared for later use. When operating, first drop 0. 5 % to l% of dicaine into the eye, then turn with the operater's left hand the upper and lower eyelids from inside out to expose the affected fornix. Stab the granules with the tip of the needle till blood is slightly exudating. Finally drop the eye with some antibiotics when the operation is finished.
Attention must be paid to the sterilization and the injury of the cornea should be avoided. If the lesion area is too large, the therapy can be divided into several treatment courses or stages.
Wild Chrysanthemum 10g, Mulberry Leaf 10g, White Sodium Sulfate 5g. Decoct them in the water and clarify the decoction, drop into the eyes for three times every day.
Silkworm Droppings (Bombycis Excrementum) 30g, Prunella Spica 50g. Decoct them in the water, then get rid off the dregs, drop into the eyes for twice or three times a day.
(2) Acupoints Massage
1) Knead Tianying (Ashi Point) gently by thumbs.
2) Press Jingming (BL 1) downward gently first, then crush upward by one thumb.
3) Knead and press Sibai (ST2) by forefinger.
4) Press Taiyang (EX- HN 5), scrape orbits in rotation: Press Taiyang (EX- HN 5) by thumbs, then scrape gently orbits one circle by second knuckle of bent forefinger, from inner upper to outer upper to outer lower to inner lower, massage Cuanzhu (BL 2), Yuyao (EX-HN4), Sizhukong (SJ23), Tongziliao (GB 1)">Tongziliao (GB 1), Qiuhou (EX-HN7), Chengqi (ST 1).
(3) Guasha Therapy
1) One the head and neck: Tongziliao (GB 1)">Tongziliao (GB 1), Yangbai (GB14), Jingming (BL1), Taiyang (EX- HN 5), Dazhui (GV14). On the back: Pishu (BL20), Weishu (BL21). On the upper limbs: Quchi (LI 11). On the lower limbs: Xuehai (SP 10), Zusanli (ST 36), Taichong (LR 3).
2) Apply some Guasha oil to the affected parts, hold the Guasha board to keep 45 degrees with the skin, and scrape the above acupoints with comfortable force from top to bottom until a local red speckle occurs.
(4) Cupping Therapy
Acupoints selected: Yangbai (GB14), Jingming (BL 1), Taiyang (EX- HN 5), Dazhui (GV14), Feishu (BL 13), Pishu (BL20), Weishu (BL21), Zusanli (ST 36), Xuehai (SP 10).
Manipulation: Apply simple cupping manipulation to the above acupoints for 10-15 minutes, once every other day or depend on the actual situation, 10 times as a course of treatment.
