It is applicable to urinary infection and calculi in urinary system with the above manifestations, which pertain to heat syndrome of the urinary bladder. It is advisable to replace No. 10 with Shenggancao (Radix Glycyrrhizae), aiming to clearing and purging away heat.
In case of excessive heat in the lower energizer, add Huangbai (Cortex Phellodendri), Pugongying (Herba Taraxaci) and Shiwei (Folium Pyrrosiae) to clear away heat and induce diuresis. In case of pain of the penis as if stabbed, add Hupo (Succinum) and Haijinsha (Spora Lygodii) to treat stranguria and relieve pain. In case of impairment of both qi and yin due to long course of illness, subtract No. 3 and No. 4 and add Dangshen (Radix Codonopsis Pilosulae), Huangqi (Radix Astragali seu Hedysari) and Ejiao (Colla Corii Asini) to supplement qi and nourish yin.
Elucidation:
The syndrome results from heat accumulation in the urinary bladder, causing impairment of the blood vessels and disturbance of qi transformation of the urinary bladder. It should be treated by removing heat from blood to arrest bleeding and inducing diuresis to treat stranguria. The first two ingredients clear away heat from blood and arrest bleeding and function as monarch drugs. No. 5 and No. 7 can not only clear away heat from blood, but also remove blood stasis, acting as minister drugs.
No. 3, No. 4 and No. 6 possess the function of clearing and purging away heat and inducing diuresis, while No. 9 has the effect of purging away fire from the triple energizer and descending heat. No. 8 can nourish yin and blood in combination with No. 1 so as to induce diuresis without impairing yin, and can prevent ingredients of cold or cool nature from causing blood stasis because it has a warm nature. These five ingredients together play the part of adjuvant drugs. No. 10 is used as guiding drug to regulate the middle-energizer and mediate drug properties.
Cautions: This recipe is contraindicated for hematuria without manifestations due to pathogenic heat.
