Diagnostic Essentials for Cellulitis (Phlegmon) in Chinese medicine:
1. Hand Phlegmon
Mostly, it happens with a history of infection due to traumatic injury, manifested by red swelling on the dorsum of the hand, lingering condition, burning pain, but without abnormality in the palm, and accompanied by aversion to cold, fever, headache and aching sensation in the body, reddened tongue, thin and yellowish coating, rapid pulse.
2. Hip Phlegmon
It is also called hip carbuncle and can be caused by infection due to unclean intramuscular injection or by extending of local boils and sores, manifested by local red swelling and burning pain, difficult walking. Red swelling is most obvious in its center and is light on the sides. Progressively, the superficial skin becomes wet and ulcerative in brown color and decayed rupture. Usually, the discharge pus is in yellow and tenacious nature and the wound can heal in about one month. It is accompanied by chills, high fever, aching pain in the body and limbs, constipation and scanty urine, reddened tongue, yellowish and greasy coating, full and rapid pulse.
3. Foot Phlegmon
It is mostly related to a history of local injury or infection of tinea pedis before sickness. In seizure, the dorsum of the foot is swelling in red color, with progressive tumefaction and unclear margin, burning pain and difficulty in walking, but without abnormality in the sole. After suppurative ruptures, purulent fluid lingers and peripheral swelling is difficult to diminish. It is accompanied by aversion to cold, high fever, peor appetite, reddened tongue,thin and yellowish coating, soft and rapid pulse. In the laboratory examination, the total count of peripheral leukocyte and neutrophil ratio are obviously enhanced in this disease.
