Clinical Application:
This recipe is indicated for yin-type carbuncle, marked by local diffuse swelling, unchanged skin color, soreness without pyrexia, absence of thirst, pale tongue with whitish fur, deep thready or slow thready pulse. It is applicable to bone tuberculosis, peritoneal tuberculosis, chronic osteomyelitis, periostitis, chronic lymphnoditis, rheumatoid arthritis, thromboangiiris obliterans, deep abscess, and others, which are ascribed to syndrome of cold accumulation due to insufficient blood.
Elucidation:
The syndrome is caused by insufficient blood due to usual deficiency of yang and attack by exogenous cold, leading to accumulation of cold and phlegm, which retain in the tissues, tendons, blood vessels and joints. This should be treated by warming yang and enriching blood, dispelling cold and removing phlegm so as to remove stagnation and subdue swelling. With a larger dosage, No. 1 is meant to nourish blood. No. 4 is supposed to replenish essence, activate the kidney-yang and strengthen tendons and bones. Both can deal with the root cause of the disease by nourishing blood and yang and function as monarch drugs.
No. 6 and No. 2 possess the function of warming the interior to dispel cold as well as ensuring smooth circulation of blood, playing the role of minister drugs. Used as adjuvant drugs, No. 3 and No. 5 can promote the flow of blood and qi on the one hand, and bring about the tonic action of No. 1 and No. 4 and prevent stagnation on the other because No. 3 is capable of removing obstruction from meridians and collaterals and No. 5 eliminating stagnation of phlegm from the muscular striae, tendons and vessles. No. 7 is the guiding drug in the recipe, which can remove toxin and mediate drug properties.
Caution:
This recipe is contraindicated for patients with yang syndrome of external carbuncle.
